The twins died in Starship Children’s Hospital on 18 June 2006. There are two parts to the findings. The twins father Chris Kahui was found not guilty of their murder on 22 July 2008. Part 1 of this report seeks to establish the causes and circumstances of their deaths as required by s15(1)(a) of the Coroners Act 1988. That act has since been repealed but applies to the proceedings of this inquest under the transitional provisions of the Coroners Act 2006. Part 2 is “given over to the phenomenon of child abuse of which this case is a subset”. The Coroner’s recommendations, directed to the Ministry of Social Development and Employment are set out in paragraph 228.
